Admission
SUMMARY
Parents are encouraged to read the
Parent's Handbook. All forms must be returned prior to the boy
attending the winter or summer program.
Request Info
Anyone interested in learning more about
our organization, the winter program, or summer camp program should fill
out and submit the
Request Info form.
Boy's Form
Interested boys should fill out the
Boy's Application Form. This will provide us with pertinent information
needed when a boy joins our program.
Medical Form
A signed medical form needs to be filled
out by the boy's physician and returned to us prior to attending the
summer program.
Parent's Form
Interested parents/guardians should fill
out the Parent's Application Form. This will provide us with pertinent
information needed when their son joins our program.
Permission/Release Form
Parents should fill out this
Permission/Release Form allowing your son to participate in our summer camp, and the entire
Berwick program. This form will also allow us to use your son's
picture, or other likeness, in any publicity and/or campaign materials.
Registration Form
All boys must have a registration form on
file.
Parent's Handbook
The
handbook
is everything you want to
know about our summer program. It also provides some background to
the winter program and our overall organization. Parent's are strongly
encouraged to read through the handbook.
Clothing/Equipment List
The
Clothing/Equipment List Clothing/Equipment List includes all items that the boys
should and should not bring to summer camp.
